THE FOLLOWING DOCUMENTS WILL BE REQUIRED FOR YOUR APPLICATION:
IDENTIFICATION: Government issued photo ID.
CITIZENSHIP STATUS: Birth Certificate for all household members, Alien Registration card(s), amnesty card(s), naturalization papers.
SOCIAL SECURITY CARD: Social Security Card or verification from SSA that you applied.
EMPLOYEE PAY STUB: Most recent, consecutive 3 month's pay stubs showing the name and address of the employee & employer.
OTHER INCOME: Award letter or Statement from Social Security, Retirement(s), Pension(s), VA Benefits, School Grants &/or Student Loan(s), Disability, Unemployment, Passport to Services from Human Service Dept., Child support.
SELF EMPLOYMENT: Last year's tax return, business records for the current and prior year, proof of business expenses.
